This is the Request for Volunteer form for Organizations wanting to partner with CESL's Student Volunteer Centre for one or more Volunteer Centre Placements.

Before You Get Started

Request Form Details

This form has 4 pages (after this one), which include:

  1. Organization Information
  2. Volunteer Placement Details
  3. Placement Contact Person(s)
  4. Liability Waiver 

The length of time to complete will vary depending on your placement. Typically, this form should take no longer than 15 minutes if you have your placement details outlined prior to beginning.
