Please use this form if you and your community-based organization and are wanting to request volunteers through the CESL's Student-Run Volunteer Centre. Before You Get Started

Request Form Details

This form has 4 pages (after this one), which include:

  1. Organization Information
  2. Volunteer Opportunity Details
  3. Placement Contact Person(s)
  4. Identified Risks and the Waiver Agreement 

The length of time to complete will vary depending on your placement. Typically, this form should take no longer than 10 minutes if you have your opportunity details outlined prior to beginning. 

If you are submitting for more than one volunteer opportunity, consider submitting each as their own request. If you submit them on one form, please be sure to be clear on the details for each opportunity. Provide all the information needed to ensure that students are fully informed of what to expect in the role.
